1、多组学综合图谱解析转移性肝癌的时空演变

摘要:目前肝癌转移肿瘤的基因突变和分子表达全景图谱仍极度缺乏,肝癌在转移过程中的克隆选择等演化规律尚不清晰,导致肝癌转移病灶的临床诊疗仍然缺乏足够的理论指导。在这里,作者对182例HCC患者的257个原发性和176个转移灶进行了多组分析。原发性和转移性HCC的基因组差异很高,早期播散很普遍。在转移瘤中观察到的显著的肿瘤内新抗原异质性与T细胞反应性降低有关,这是由于新抗原递呈中断所致。没有Wnt突变的亚克隆比有Wnt突变的亚克隆显示出更强的转移选择性优势,并以微环境中丰富的活化成纤维细胞为特征,有利于前转移表型。最后,没有Wnt突变的转移瘤表现出更高的免疫抑制B细胞富集,通过HLA-E:CD94-NKG2A检查点轴介导CD8+ T细胞的末端衰竭。总的来说,作者的结果提供了一个复杂的转移进化过程的多维解剖。
