降钙素基因相关肽家族,包括降钙素(CT, Calcitonin)、胰岛淀粉样肽(AMY, Amylin)、降钙素基因相关肽(CGRP, Calcitonin Gene Related Peptide) 、肾上腺髓质素(AM, Adrenomedullin) 。其中,人的α-CGRP,β-CGRP蛋白质之间相差3个氨基酸。


降钙素基因相关肽家族的受体分为两种亚型:结合CT的降钙素受体(CALCR、CTR)和降钙素受体样受体(calcitonin receptor–like receptor, CRLR、CLR、CALCRL)。其中,降钙素基因相关肽(CGRP)受体由降钙素受体样受体、受体活性修饰蛋白(RAMP1)和CGRP受体成分蛋白(CRCP)三部分组成。 RAMP1是降钙素样受体运输到质膜和受体与CGRP激动剂偶联所必需的。 CRCP参与了CGRP受体和G蛋白之间的联系。 配体结合后,复合物与G蛋白相互作用,Gαs亚基从复合物中分离并激活腺苷酸环化酶,产生cAMP。 细胞内cAMP浓度的升高刺激cAMP依赖的信号通路,最终导致转录因子cAMP反应元件结合蛋白与启动子结合,诱导基因表达。


临床前证据表明,在偏头痛期间,三叉神经节中激活的感觉神经元从脑膜内的投射神经末梢释放CGRP。 释放的CGRP结合并激活CGRP受体,引起血管舒张和血浆外渗。因此,在偏头痛的治疗领域中。CGRP拮抗剂逐渐崭露头角。
